A guided life review and legacy planning journal — for the stories that deserve to be told, the lessons worth passing down, and the people who will carry them forward.
Move through them in order, or open whichever one calls to you first. There is no wrong place to start.
The moments that made you who you are — childhood, turning points, the parts of your story only you can tell.
What you learned the hard way, the easy way, and every way in between — wisdom worth handing down.
The beliefs and principles that guided your choices, and what you hope outlives you.
Words for the people you love, written now so they don't have to wait.
Recipes, traditions, and memories worth preserving for the people who come after.

Horizon Legacy is designed to sit alongside the work care teams are already doing.
Make space for life review during a hospice stay, alongside chaplaincy, social work, or volunteer visits. Horizon Legacy gives patients and families a structured way to put a life into words — often surfacing the conversations that matter most.
Life review is part of whole-person care. Offer Horizon Legacy as a complement to symptom management and goals-of-care conversations — something patients can return to between visits, at their own pace.
Build legacy work into life enrichment, pastoral care, or family programming. Horizon Legacy gives residents a dignified, structured outlet to record their stories — independently, or with help from staff, family, or volunteers.
Horizon Legacy can be completed independently, at a person's own pace, or facilitated by a chaplain, social worker, volunteer, or family member — reading prompts aloud and writing down what's shared. There is no required order and no wrong way to use it. Some chapters take ten minutes; others take ten conversations.
Reflections are stored only in this browser, on this device — nothing is uploaded, transmitted, or shared with Health Horizon or anyone else. To keep a copy permanently, use the "Review & Print" option to save it as a PDF or print it before closing the browser.
